Afloat Class A Mishaps

Ship and Submarine Mishaps

  • 14 Feb 2013: (Naval Base Yokosuka, Japan) CG sustained Class B fire in main engine room while moored pier side.
  • 14 Feb 2013: (near Catalina Island, CA) During submarine rescue system training, the Pressurized Rescue Module (PRM) fell damaging the PRM, transfer skirt, deck cradle, and umbilical.
  • 17 Jan 2013: (Philippines) MCM ran aground operating in the Sulu Sea near Tubbataha Reef.
  • 09 Jan 2013: (Strait of Hormuz) SSN sustained damage to periscope during transit.
  • 12 Dec 2012: (Cherry Point OpArea) E-3 sustained head injury when shoring fell while working in well deck.
  • 13 Oct 2012: (Atlantic Ocean off the Coast of Jax, FL) Cruiser and sub collided.

 

Diving Mishaps

  • 26 Feb 2013: (Aberdeen, MD) During unit-level training final evaluation problem dive, two E-5's were recovered unconscious and unresponsive.  One died on scene and the other diver was pronounced dead at the hospital.
