Please send the following in an email to SAFE-Code15@navy.mil  to request a Culture Workshop.  If you do not receive a reply within 3 working days, please contact the Culture Workshop Division Head.

1.  POC name, phone number (comm and DSN), and email address

2.  CO's name, phone number (comm and DSN), and email address

3.  Unit UIC

4.  T / M / S

5.  Location of workshop

6.  Primary and secondary dates for workshop (two day process)

7.  Additional information if required
