The FSGLI Web Application is a DMDC hosted online system for managing FSGLI accounts.  Generally, all Sites and Users have been arranged under Regional PSD or RCC.  Certain other major entities (commands and vessels) have SSM level privileges as well.  Each of these sites must have a Primary Site Security Managers (PSSM) and Secondary Site Security Manager (SSSM). 

Personnel designated to perform FSGLI transactions must submit a fully authenticated System Authorization Access Request - Navy (OPNAV Form 5239/14) to
the SSM responsible for their reviewing their request and providing access. An example of the SAAR-N is provided in the links on this page. The SAAR-N must be signed by the requestor and supervisor. The Command Information Assurance Manager must complete Blocks 21 - 24. The Security Manager (with access to JPAS) must complete all of Part III.

ALL USERS OF THE FSGLI WEB APPLICATION ARE REQUIRED TO BE IT LEVEL II COMPLIANT (LEVEL II or LEVEL I), SEE DODI 8500.2  and DODD 8500.1 FOR REQUIREMENTS.

Upon receipt of SAAR-N, SSM will review for completion and compliance with security standards. Once reviewed and approved, SSM will provide access to the requestor, thus making them a FSGLI Site User. Site Users will be responsible for entering / processing FSGLI coverage changes on the new Website. Your SSM will provide you with a FSGLI Site User Guide.
