THE SURVEY CAN BE COMPLETED ON-LINE (STARTING 10 APR) AT NAVPERSCOM'S WEB SITE, WWW.BUPERS.NAVY.MIL, BY SELECTING THE "PERS-40 CUSTOMER SATISFACTION" HOT KEY. THE SURVEY SHOULD TAKE ABOUT 10 MINUTES TO COMPLETE; PARTICIPANTS MUST TYPE IN THE CORRECT PASSWORD (PERS-40CS) AND THE LAST FOUR DIGITS OF THEIR SSN. 4. THE SURVEY PERIOD WILL COVER FROM 10 APRIL 00 TO 12 MAY 00, AND ONLY DATA AVERAGED ACROSS THE ENLISTED COMMUNITY WILL BE MADE PUBLIC. INDIVIDUAL RESPONSES TO THE SURVEY QUESTIONS WILL REMAIN CONFIDENTIAL.
