Mobilization Readiness Review
THIS IS NOT A MOBILIZATION/ACTIVATION NOTICE
As a member of the IRR, you are required by statute to demonstrate your medical readiness. As per 10 U.S.C. § 10206:
“Each Reserve in an active status [which includes the IRR], or on an inactive status list, who is not on active duty shall execute and submit annually to the Secretary concerned a certificate of physical condition.”
Establishing and maintaining the highest levels of readiness ensures Navy’s compliance with 10 U.S.C. § 10149 which directs regular and continuous screening of the Ready Reserve so that “[t]hat there will be no significant attrition of those members or units during a mobilization.”
You must complete and return a Report of Medical History (DD Form 2807-1) within 45 days of receipt of initial communication from PERS-93.
NOTE: There will be NO reimbursement for expenses incurred when complying with the 2807 Requirement.
Failure to do so will result in negative administration up to and including separation from the Naval service. The form must be reviewed and signed by a medical provider (civilian or military physician). Ensure that the examiner provides his or her phone number in Block 5 and that your current height/weight is recorded in Block 29.
Once completed, the form can be sent to PERS-93 in any of three methods: 1. Scan and email to PERS-9_muster@navy.mil 2. Fax to (901) 874-2363 (ensure you use a cover sheet) 3. Regular mail to:
