Send PERS-81 conversion requests via email to  

  • All packages must include:
         -Copy of member's C-WAY for E6 and below (do not submit via C-WAY)
         -Copy of member's PFA (4yr history)
         -Copy of last three evaluations
         -Copy of disqualifying documentation if Forced Conversion                                           
  • All ratings requiring TS and TS/SCI clearances must have a current SF86 (within last 30 days and must be signed) and SSO interview included in the package(SF 86 NOT REQUIRED IF MEMBER ALREADY HAS A TS CLEARANCE).
