• Conversion opportunity for ERB Sailors NAVADMIN 160/11
   
• Special request procedures contained within NAVADMIN 160/11.
• Rates listed under NAVADMIN 160/11 paragraph 3 are the only rates offered under this conversion opportunity for ERB eligible Sailors.
• Ensure NAVADMIN 160/11 paragraph 5 special conditions of understanding are on request.
• Enlisted Retention Board (ERB) NAVADMIN 129/11
  
  
• PERS-811 processes non-PTS lateral, direct, and forced rate conversions along with striker removals IAW MPM 1440-010 for active duty and FTS Sailors.  We do not handle:
  
  
• PTS conversions: (901) 874-2102 or DSN: 882-2102; Email pts_help_desk@navy.mil
  
• Undesignated Sailors who desire "A" School - Contact PERS-4010 or call 1-866-U-ASK-NPC (1-866-827-5672 DSN 882-5672)
  
• Special Warfare rating conversions - should be submitted IAW their corresponding MPM articles.

  ND - MPM 1220-100;  EOD - 1220-200; SO - 1220-300; SB - 1220-400

• Send PERS-81 conversion requests via email to mill_conversionsP811E@navy.mil or fax to (901) 874-2043/2044; DSN 882-2043/2044.  All packages require documentation, therefore requests submitted via BUPERS ONLINE (BOL) will be rejected.

• All packages must include:

-Copy of member's FLEET RIDE for E6 and below (do not submit via FLEET RIDE)

-Copy of member's PFA (4yr history)

-Copy of last three evaluations

-All ratings requiring TS and TS/SCI clearances must have a current SF86 (within last 30 days and must be signed) and SSO interview included in the package.

• SF86 must be this latest form (ver Jul 2008) 
• Sample SSO Interview

Submit SF86 as enclosure to conversion package, do not submit to DONCAF to initiate background investigation.
