Send PERS-81 conversion requests via email to mill_conversionsP811E@navy.mil

All packages must include:
     -Copy of member's FLEET RIDE for E6 and below (do not submit via FLEET RIDE)
     -Copy of member's PFA (4yr history)
     -Copy of last three evaluations
     -All ratings requiring TS and TS/SCI clearances must have a current SF86 (within last 30 days and must be signed) and SSO interview included in the package.
