Federal employees are required to have their pay deposited directly into a financial institution account. Please complete Sections 1 and 2 of Form SF-1199A and bring it to your financial institution for completion of Section 3.
In Section 1, Block C, use your Social Security Number.
In Section 1, Block F, mark the box "Fed. Salary/Mil. Civilian Pay".
In Section 1, Block G, leave this block blank.
In Section 2, fill in the name and address where you will be working.
Bring the completed form with you on your first day of work.
Please Note: In some circumstances it may take up to 4 weeks for your first paycheck to be deposited into your account. You will receive a statement each pay period beginning with your first direct deposit.
