 FEGLI CLAIM FOR BENEFITS/DEPENDENT DEATH

Claims for benefits are adjudicated and paid by the Office of Federal Employees' Group Life Insurance (OFEGLI). The Civilian Benefits Center provides counseling and helps survivors file claims for benefits.

 ACTION REQUIRED TO FILE A CLAIM FOR BENEFITS DUE TO DEATH OF A DEPENDENT

  1. You should contact the Benefits Line for counseling and to obtain an FE-6DEP, Statement of Claim – Option C Family Life Insurance form to file for benefits.
  2. Complete the FE-6DEP and send it to the Civilian Benefits Center with a certified copy of the death certificate.
  3. The Civilian Benefits Center will complete Part D – Employing Agency/OPM Certification of Insurance Status and mail the application package to OFEGLI.
  4. OFEGLI will process the claim once the application package is received from the Civilian Benefits Center.
