1. Is your legal, not common law, spouse listed in DEERS/RAPIDS?
NO. You must register your spouse in “DEERS/RAPIDS” to qualify for FSGLI.
YES. Coverage is automatic starting 01 NOV 2001 or date of marriage, whichever is later.
Note: Failure to register in “DEERS/RAPIDS” will incur a debt and will continue to increase until the Servicemember's spouse is registered in DEERS.
2. Did the Servicemember refuse SGLI coverage?
NO. Servicemember is automatically covered starting 01 NOV 2001 or date of marriage, if later.
YES. Servicemember is not eligible for FSGLI, spouse or child. For coverage complete a SGLV 8286A Family Coverage Election or SGLV 8285A Request for Family Coverage for Spouse (SGLI) for reinstatement.
Note: If you don’t want spouse coverage submit a SGLV-8286A. You must hand write “I DO NOT WANT COVERAGE FOR MY SPOUSE AT THIS TIME”. Make sure the form is signed, dated and the witness signs and dates the form. The witness is certifying your signature so all dates should be the same. The DMDC clerk will then make the change and file the form in your service record. Ensure you maintain a copy for your records.
3. Did the Servicemember complete the SGLV 8286A Family Coverage Election to reduce or terminate FSGLI?
NO. Coverage is automatic and remains in effect. The Servicemember is liable for premium payments starting 01 NOV 2001 or date of marriage whichever is later.
YES. Termination or reduction is effective the first day of the month after the date SGLV 8286A is signed and witnessed.
